Responsibilities
- Personal Care Assistance: Assist patients with daily living activities including bathing, dressing, grooming, and toileting to ensure personal hygiene and comfort.
- Medication Management: Remind patients to take prescribed medications on time and report any adverse reactions or changes in condition to the supervising nurse.
- Meal Preparation: Prepare nutritious meals and snacks according to specific dietary restrictions and patient preferences.
- Light Housekeeping: Maintain a clean, safe, and organized living environment, including laundry and light cleaning duties.
- Companionship & Social Engagement: Engage patients in conversation, hobbies, and social activities to reduce isolation and improve mental well-being.
- Vital Monitoring: Track and accurately record vital signs such as blood pressure, pulse, and temperature.
- Transportation Support: Accompany patients to medical appointments, therapy sessions, or errands as required.
- Documentation: Maintain accurate and up-to-date patient care records and logs.
Qualifications
- Education: High School Diploma or GED is required.
- Certification: Current and valid California Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license.
- Experience: Minimum of 1 year of experience in home health or personal care is preferred.
- Driver's License: Valid driver's license and reliable transportation are required.
- Skills: Strong interpersonal communication skills and a genuine, compassionate desire to help others.
- Physical Requirements: Ability to lift, bend, stand, and walk for extended periods; ability to assist with transfers.
- Clean Record: Must pass a background check and drug screen.
