Responsibilities
- Provide direct personal care assistance to patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and record patient vital signs, such as temperature, pulse, and respiration rates, reporting any significant changes to the Registered Nurse.
- Assist with light housekeeping tasks, meal preparation, and medication reminders as directed by the case plan.
- Offer emotional support and companionship to patients to enhance their quality of life and mental well-being.
- Transport patients to medical appointments or recreational activities using a personal vehicle or company vehicle.
- Document daily care activities and patient progress accurately in electronic health records (EHR) systems.
- Adhere to strict HIPAA privacy regulations and maintain a clean, safe, and hygienic environment for patients.
Qualifications
- High School Diploma or GED equivalent required.
- Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is strongly preferred.
- Current valid Driver's License and reliable transportation.
- CPR and First Aid certification (or willingness to obtain within 30 days of hire).
- Excellent communication skills and a genuine, compassionate demeanor towards the elderly and disabled.
- Ability to lift, bend, and stand for extended periods as required by the job duties.
- Pass a background check and drug screening.
