Responsibilities
- Provide personal care assistance including bathing, dressing, and grooming
- Prepare and serve nutritious meals as per care plans
- Monitor patient vital signs and report changes to clinical staff
- Assist with mobility, transfers, and range-of-motion exercises
- Perform light housekeeping tasks and medication reminders
- Transport patients to appointments and errands
- Maintain accurate care documentation
Qualifications
- Minimum 6 months of home health or caregiving experience
- Valid California Driver's License and reliable transportation
- Certified Nursing Assistant (CNA) or Home Health Aide certification preferred
- CPR/First Aid certification (or willingness to obtain)
- Ability to pass background check and TB screening
- Strong communication and interpersonal skills
