Responsibilities
- Conduct Patient Assessments: Utilize clinical judgment to evaluate patient symptoms via phone, secure messaging, or video calls to determine the appropriate level of care.
- Provide Medical Triage: Offer evidence-based advice on home care treatments, medications, and when to seek emergency services.
- Facilitate Referrals: Coordinate appointments with physicians, specialists, or urgent care centers as necessary.
- Document Care: Accurately record patient interactions, triage decisions, and care plans in the Electronic Health Record (EHR) system.
- Collaborate with Providers: Communicate critical findings and patient status updates to the care team to ensure continuity of care.
Qualifications
- Active RN License: Must possess a current, unencumbered Registered Nurse license in the state of Texas.
- Certifications: BLS (Basic Life Support) and ACLS certification required.
- Experience: Minimum of 2 years of clinical nursing experience, with 1 year specifically in triage or telehealth.
- Communication Skills: Excellent verbal and written communication skills with a focus on empathy and clarity.
- Technology Proficiency: Comfortable navigating EMR systems and remote patient monitoring tools.
