Responsibilities
- Conduct comprehensive patient assessments and monitor vital signs to ensure optimal health outcomes.
- Administer medications and treatments in accordance with state licensure and facility protocols.
- Provide telehealth consultations and remote triage for patients in the San Jose area.
- Collaborate with physicians and multidisciplinary teams to develop individualized care plans.
- Document patient history, progress notes, and treatment plans accurately in electronic health records (EHR).
- Educate patients and families on disease management, medication adherence, and preventive care.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of California.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Minimum of 2 years of clinical nursing experience preferred (Telehealth experience is a plus).
- Strong communication skills with the ability to interact effectively with patients and staff.
- Proficiency in electronic health record systems (EHR) such as Epic or Cerner.
- Ability to work independently in a remote environment while maintaining strict confidentiality.
