Responsibilities
- Conduct thorough patient symptom assessments via phone or in-person to determine the level of urgency and appropriate level of care.
- Coordinate patient referrals to appropriate medical providers, specialists, or urgent care facilities.
- Document patient interactions, assessments, and care plans accurately and efficiently within the Electronic Health Record (EHR).
- Provide patient education regarding symptoms, preventative care, and medication management.
- Collaborate effectively with physicians, nurse practitioners, and other clinical staff to ensure seamless continuity of care.
- Manage high-volume patient inquiries with a high degree of empathy, patience, and clinical accuracy.
Qualifications
- Licensure: Active and unencumbered Registered Nurse (RN) license in the State of California.
- Experience: Minimum of 2-3 years of recent experience in clinical triage, emergency nursing, or urgent care.
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Skills: Strong clinical judgment, critical thinking, and decision-making abilities.
- Communication: Excellent verbal and written communication skills with the ability to de-escalate patient concerns.
- Technology: Proficiency in using EMR/EHR systems (e.g., Epic, Cerner, Athenahealth).
