Responsibilities
- Perform comprehensive telephone and secure messaging triage assessments for patients reporting symptoms.
- Evaluate patient symptoms to determine the appropriate urgency level and recommend appropriate care pathways.
- Coordinate timely referrals to primary care providers, specialists, or urgent care facilities.
- Document all patient interactions, assessments, and care plans accurately and securely in the EMR.
- Collaborate with physicians and care management teams to ensure continuity of care.
- Provide clear health education and self-care instructions to patients and families.
- Monitor patient feedback and assist in the continuous improvement of triage protocols.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of California.
- Certification in BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) is required.
- Minimum of 2 years of recent clinical experience in emergency nursing, urgent care, or triage.
- Proficiency in using electronic health records (EMR) and telehealth platforms.
- Strong critical thinking and decision-making skills under pressure.
- Excellent verbal and written communication skills with a patient-centered approach.
