Responsibilities
- Perform rapid and accurate triage assessments of patient symptoms via telephone and secure messaging.
- Utilize evidence-based protocols to determine the urgency of patient needs and recommend appropriate care pathways.
- Document detailed patient histories, symptoms, and triage outcomes accurately in the EMR system.
- Collaborate with physicians, advanced practice providers, and other healthcare team members to coordinate seamless patient care.
- Provide patient education and resources to promote self-care and wellness.
- Manage high-volume call queues efficiently while maintaining a calm and professional demeanor under pressure.
- Participate in quality improvement initiatives to enhance triage protocols and patient outcomes.
Qualifications
- Current and valid Registered Nurse (RN) license in New York State (NYS).
- Minimum of 2 years of nursing experience, with at least 1 year in a triage or telephone nurse case management role preferred.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong computer literacy and proficiency with Electronic Health Records (EMR) systems.
- Excellent verbal communication skills with the ability to de-escalate situations and build rapport with patients.
- Ability to think critically and make sound clinical judgments in real-time.
