Responsibilities
- Perform virtual patient assessments and monitor vital signs remotely using secure telehealth platforms.
- Document patient interactions, care plans, and medical updates accurately in EHR systems.
- Communicate effectively with physicians, nurses, and family members regarding patient status.
- Provide health education and guidance to patients to ensure adherence to treatment plans.
- Maintain strict confidentiality and compliance with HIPAA regulations.
- Coordinate follow-up appointments and referrals for patients.
- Collaborate with the multidisciplinary care team to optimize patient outcomes.
Qualifications
- Current, valid Certified Nursing Assistant (CNA) license in the State of Ohio.
- Previous experience in home health, hospice, or telehealth coordination is highly preferred.
- Proficient computer skills with the ability to navigate electronic health record software.
- Strong written and verbal communication skills.
- Ability to work independently with minimal supervision in a remote environment.
- Reliable high-speed internet connection and a dedicated home workspace.
- Current BLS (Basic Life Support) certification.
