Responsibilities
- Coordinate patient care plans and telehealth appointments remotely
- Provide virtual support to patients with chronic conditions and post-acute care needs
- Document patient interactions accurately in our HIPAA-compliant system
- Collaborate with RNs and physicians to ensure continuity of care
- Monitor patient progress and escalate critical issues promptly
- Conduct virtual wellness checks and medication reminders
- Maintain strict confidentiality and compliance with healthcare regulations
Qualifications
- Active CNA certification in New York State
- High school diploma or equivalent
- Basic computer proficiency and reliable internet connection
- Strong communication and empathy skills
- Ability to work independently with minimal supervision
- No prior telehealth experience required
- Background check and drug screening clearance
- Willingness to complete our virtual training program
