Responsibilities
- Conduct Comprehensive Patient Assessments: Utilize evidence-based protocols to evaluate patient symptoms via phone or secure messaging, determining the urgency of care required.
- Prioritize Care and Facilitate Triage: Accurately categorize patient needs (Urgent, Non-Urgent, Emergency) and direct them to the appropriate care pathway.
- Document Care Accurately: Maintain detailed and up-to-date electronic health records (EHR) for every patient interaction to ensure continuity of care.
- Collaborate with Care Teams: Communicate effectively with physicians, nurse practitioners, and case managers to ensure coordinated patient management.
- Manage High-Volume Call Queues: Efficiently handle a high volume of incoming inquiries while maintaining a calm and professional demeanor.
- Patient Education: Provide clear, understandable instructions regarding home care, medications, and follow-up appointments.
Qualifications
- Licensure: Current and active Registered Nurse (RN) license in the state of Georgia (GA).
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Experience: Minimum of 2-3 years of experience in emergency nursing, urgent care, or telephone triage is preferred.
- Communication Skills: Excellent verbal communication skills with the ability to explain complex medical information clearly to patients of all backgrounds.
- Technical Proficiency: Comfortable using Electronic Health Records (EHR) systems and telehealth platforms.
- Decision Making: Strong critical thinking and clinical judgment skills under pressure.
