Responsibilities
- Conduct thorough assessments of patient symptoms via phone and in-person interactions to determine urgency and appropriate care pathways.
- Manage a high volume of incoming calls with accuracy, empathy, and efficiency.
- Document patient interactions and triage decisions accurately in the Electronic Medical Record (EMR).
- Provide patient education and self-care instructions to promote recovery and prevent readmission.
- Collaborate with physicians and other healthcare providers to ensure seamless continuity of care.
- Assist in the coordination of referrals to specialists or urgent care facilities as needed.
- Monitor patient outcomes and follow up to ensure satisfaction and proper care delivery.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the state of Missouri.
- Minimum of 2 years of clinical nursing experience, with specific experience in triage or telehealth preferred.
- BLS and ACLS certifications (or willing to obtain).
- Strong computer skills, including proficiency with EMR systems (Epic, Cerner, AthenaHealth).
- Excellent communication and interpersonal skills.
- Ability to remain calm and professional in high-stress situations.
