Responsibilities
- Perform comprehensive and accurate patient symptom assessments via telephone, video consult, or in-person visits.
- Apply clinical reasoning to determine the urgency of a patient's condition and route them to the appropriate level of care.
- Educate patients on self-care management, medication adherence, and preventative health measures.
- Document all interactions and patient data accurately into the Electronic Health Record (EHR) system.
- Collaborate effectively with physicians, care coordinators, and other healthcare professionals to ensure seamless continuity of care.
- Maintain strict adherence to HIPAA regulations and company privacy policies.
- Monitor and report on key performance indicators related to call handling and patient satisfaction.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of Arizona.
- Minimum of 2-3 years of acute care or triage nursing experience.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong computer proficiency with experience navigating EMR/EHR software.
- Excellent verbal and written communication skills with a focus on patient empathy.
- Ability to work independently while adhering to established protocols and schedules.
