Responsibilities
- Conduct comprehensive initial assessments of patient symptoms via phone or in-person to determine medical urgency.
- Utilize clinical decision-making skills to prioritize patient care and determine the appropriate level of care required.
- Document all triage interactions, patient findings, and care plans accurately and timely into the Electronic Health Record (EHR).
- Collaborate effectively with physicians, advanced practice providers, and nursing staff to ensure seamless care coordination and patient handoffs.
- Provide clear, empathetic, and actionable health education and instructions to patients and families regarding self-care and next steps.
- Monitor patient flow and assist in the efficient operation of the triage unit during peak hours.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 2-3 years of recent experience in emergency nursing, critical care, or telehealth triage.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification required.
- Strong verbal and written communication skills with the ability to de-escalate situations and build rapport with patients.
- Proficiency in using EMR/EHR systems and medical terminology.
- Ability to work flexible hours, including nights, weekends, and holidays.
