Your Role
We are looking for a problem-solver who thrives in a fast-paced environment. As a Triage Nurse, you will be responsible for evaluating patient needs, providing medical advice, and ensuring seamless transitions to appropriate care facilities.
Responsibilities
- Conduct Comprehensive Assessments: Evaluate patient symptoms via phone or video to determine the urgency of care required.
- Clinical Decision Making: Utilize clinical guidelines to recommend self-care, over-the-counter medications, or immediate emergency services.
- Documentation: Accurately and timely document all patient interactions, assessments, and care plans in the Electronic Health Record (EHR).
- Referral Management: Facilitate referrals to specialists or primary care physicians when necessary.
- Patient Education: Educate patients on preventative health measures, medication adherence, and chronic disease management.
- Collaboration: Work closely with physicians and care coordinators to ensure continuity of care.
Qualifications
- Licensure: Valid and active Registered Nurse (RN) license in New York State.
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Experience: Minimum of 2-3 years of experience in emergency nursing, telemetry, or urgent care.
- Communication: Excellent verbal and written communication skills with a focus on patient-centered care.
- Technical Proficiency: Comfortable navigating EMR systems and telehealth software.
- Critical Thinking: Ability to remain calm and make sound clinical decisions under pressure.
