Responsibilities
- Patient Assessment: Conduct thorough and accurate symptom assessments via phone or in-person to determine the severity of patient conditions.
- Priority Triage: Prioritize patient needs effectively, directing patients to the appropriate level of care, including urgent care, emergency departments, or home health services.
- Care Coordination: Collaborate with physicians, nurse practitioners, and care managers to develop treatment plans and ensure continuity of care.
- Documentation: Maintain precise and up-to-date electronic health records (EHR) documenting patient interactions, assessments, and triage decisions.
- Referral Management: Facilitate timely referrals to specialists and external services as needed.
- Education: Provide patient and caregiver education regarding self-care, medication management, and follow-up instructions.
Qualifications
- Licensure: Current, unencumbered Registered Nurse (RN) license in the State of Minnesota.
- Experience: Minimum of 2-3 years of clinical nursing experience, with a strong preference for experience in urgent care, emergency nursing, or telephone triage.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Communication: Excellent verbal and written communication skills with the ability to de-escalate patient concerns and provide clear medical instructions.
- Technology: Proficiency with Electronic Health Records (EHR) systems and computer technology.
