Responsibilities
- Provide remote patient assessments and care coordination
- Monitor vital signs and document health status electronically
- Collaborate with interdisciplinary care teams
- Assist with medication management reminders
- Deliver emotional support to patients and families

- Maintain accurate electronic health records
Qualifications
- Active California CNA certification required
- Minimum 1 year home care or hospice experience
- Basic computer proficiency for telehealth platforms
- Valid CPR/BLS certification
- Clear background check and TB test
- Ability to work independently with minimal supervision
