Responsibilities
- Deliver virtual patient assessments and health monitoring via telehealth platforms
- Coordinate care plans with RNs and physicians for weekend patients
- Provide remote medication education and support to patients/families
- Document patient interactions accurately in EHR systems
- Respond to urgent patient inquiries during weekend shifts
- Facilitate seamless transitions between care settings
- Maintain strict HIPAA compliance for all virtual interactions
Qualifications
- Active California CNA license in good standing
- Minimum 1 year clinical experience in acute or long-term care
- Proficient with telehealth software (e.g., Zoom Health, Doxy.me)
- Strong written communication skills for virtual patient interactions
- Ability to manage multiple patients simultaneously during shifts
- Weekend availability (Saturday/Sunday) required
- Valid CPR/BLS certification
