Responsibilities
- Provide direct patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and report changes to nursing staff
- Assist with meals and ensure proper hydration
- Document patient observations accurately in medical records

- Maintain a clean and safe patient environment

Qualifications
- Current Wisconsin CNA certification required
- Minimum 1 year long-term care experience preferred
- Basic Life Support (BLS) certification
