Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Provide non-medical personal care and companionship to reduce isolation.
- Monitor client health status, reporting any changes or emergencies to the Case Manager immediately.
- Assist with mobility, transfers, and light exercises as prescribed.
- Prepare nutritious meals and assist with light housekeeping to maintain a safe environment.
- Administer medication reminders and document care logs accurately.
- Communicate effectively with patients, families, and the healthcare team.
Qualifications
- Valid California Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification.
- Current Basic Life Support (BLS) and CPR certification.
- Valid California Driver’s License and reliable personal transportation.
- Pass a criminal background check and drug screening.
- Strong interpersonal communication and problem-solving skills.
- Physical ability to lift, bend, and stand for extended periods.
- Willingness to work in diverse residential settings.
