Responsibilities
- Provide personalized in-home care including bathing, dressing, and meal preparation
- Monitor patient vital signs and report changes to healthcare team
- Assist with mobility transfers and light housekeeping duties
- Administer medications as prescribed and maintain accurate records
- Offer companionship and emotional support to enhance patient wellbeing
- Coordinate telehealth appointments and digital health documentation remotely
Qualifications
- Wisconsin state HHA certification or CNA license
- Minimum 1 year home care experience preferred
- CPR/First Aid certification required
- Reliable transportation and valid driver's license

- Strong communication skills and ability to work independently
- Background check and drug screening clearance
