Responsibilities
- Provide personal care assistance (bathing, dressing, grooming) to homebound patients
- Monitor vital signs and document patient health status via our digital platform
- Assist with medication management and meal preparation
- Transport patients to medical appointments using company-provided vehicle
- Companionship and emotional support for elderly and disabled clients
- Collaborate with care teams through remote telehealth sessions
- Maintain accurate electronic health records (EHR) documentation
Qualifications
- State-certified Home Health Aide or Nursing Assistant
- Minimum 1 year professional caregiving experience
- Current CPR/First Aid certification
- Reliable transportation and valid Wisconsin driver's license
- Proficient with telehealth technology and EHR systems
- Ability to work independently while adhering to care protocols
- Clear background check and drug screening required
- Empathetic communication skills with diverse populations
