Responsibilities
- Conduct comprehensive patient assessments via phone and telehealth platforms
- Prioritize cases based on clinical urgency using established protocols
- Coordinate with physicians, specialists, and ancillary departments
- Maintain accurate electronic health records (EHR) documentation
- Educate patients on care pathways and resource utilization
- Collaborate with multidisciplinary teams to ensure continuity of care
- Participate in quality improvement initiatives and process optimization
Qualifications
- Active RN license in Wisconsin (or compact state)
- Associate's or Bachelor's degree in Nursing (ADN or BSN)
- Strong critical thinking and decision-making abilities
- Excellent written and verbal communication skills
- Ability to work efficiently in fast-paced environments
- Proficiency with electronic health record systems
- Certification in Basic Life Support (BLS) required
